when you over tighten the bolt or nut it slips back over the tred and the best way to prevent it is becareful as u tighten, when wat ever ur screwing is tighten enough, and doesnt wobble, it should be good,

Seriously, though, overtightening or cross threading it will strip it, although you'll probably strip the hole out first. To keep from cross threading, insert the bolt flush, turn it as if you are LOOSING it until you feel it click or drop into the thread of the hole, then tighten it to the correct torque rating.
