There is a link on the bottom of the page under additional forms where you can apply to change your name.
Go to your courthouse and they will take you through the steps. Each state is different depending on the reasons.

It depends on what state you live in. Each state may have different procedures.
For example, here in Hawaii, you need to go through the Lieutenant Governor's Office to request to change your name. You need to complete a petition to request to change your name and upon approval of the Lieutenant Governor, you must request to publish your Notice of Name Change in a local newspaper agency for publication within 60 days. Then the Affidavit of publication must be mailed from the newspaper agency to the Lieutenant Governor's Office. After the Lieutenant Governor signs the administrative Order and it is mailed to you. Then you must mail the Order to the Bureau of Conveyances within 60 dyas of it being signed.
