you should find a subject you like and get involved. Set your goals and work hard to fulfill them. I put a picture on my bedroom wall that represents my goal and every time i feel that i can't study I look at it and i ask myself what I am willing to do to get there.

First pick the subject your are best at . think for a while, why you are relatively good in it? Try and replicate the same in your worst subject. There is nothing more stimulating when you start to see your efforts turning into results.
Second most important thing is determination and patience. Once you make a plan, stick to it till the desired results come out. It may be delayed but, if you stick to it, the results will come. But if you keep changing your plan then, you will achieve nothing but day dreams. It is not important to have the best plan. Because success comes from proper implementation.
Third, target something easily achievable. Remember: "Aim small, miss small". Once it is achieved, again set higher targets. Example - 2 page of history or 20 maths a day. once you are used to then go for 3 pages or 30 maths.
fourth - Do not memories anything .. Take your time to read from first to last and then repeat several times. You'll see each repetition takes shorter time than previous, because you've started to remember it.
