I would go with the third one, it sounds straight forward, the second sounds wrong to me and the first a longer winded version of the third.
Actually someone at the tax office has a brain, you dont have to send any letter with the tax return just the form itself but make sure you send it recorded delivery in case you need proof of the date it was posted. They know how to process the form once they receive it - humans work there as well and I would really put PLEASE FIND ENCLOSED THE SELF ASSESSMENT TAX RETURN AS REQUESTED, maybe I have done too many letters over my life, must be around 10,000 or more.
If you actually speak to someone there very nicely they will even give you the ways in which to save yourself paying tax - legally. Sorry that nothing to do with the question
