Could you contact your old school and ask for a reference? They may also be able to tell you where to go to prove your qualifications too. Have you had any job (even a Saturday one) which can give you a reference? They ask for these two things, not to prove that you're interested but to prove that you are capable and reliable - if you can get any kind of reference from people who can verify this (teacher from school, employer) then that should go a long way to meeting those requirements. Proving you're really interested will come from you in the interview - be keen, interested, ask intellegent questions, and friendly. Try reading up on new treatments which you can mention (eg "I read that there's a new treatment for ... What do you think of it? Does the course offer training in this?). And tell them what you've told us- that you want to to something for yourself and that you would relish the opportunity to do the course - they'll want to hear that as much as anything!
