I am a French teacher who also speaks english, spanish, danish and a little urdu. Practice with people everyday. Learn a new useful phrase everyday and USE it often. Writing and readng will not teach you to speak or listen they are processed by s different part of the brain. After 15 years of study, I could red and write French very well, but it was not until I spent a week in Paris that I became suddenly FLUENT! when you are forced to speak (don't be shy) you may not think so, but others think it's cute-probably. especially please, thank you and You have been very helpful can often start a conversation that will be easy and non threatening, since you are already asking for help and have found a nice person. In three days I was dreaming in French!
read a lot of books to improve your english
just practise talking to people at home and read alot of books.It helped me.

believe me, my English wasn't good either, but after i read some book, I feel more confident when i speak to someone.
Watching movies especially British (no offense intended on other "English" speaking countries e.g. USA) they will give you good pronunciation; also adverts on the net are good for learning pronunciation. Then you can practice what you have heard. Practice is key.
