I recomend getting a medium sized makeup bag, and you can put things like a hairbrush, a little makeup (like mascara or lip gloss), tampons/pads, some extra change (a dollar in quarters for example), extra pens/pencils/hi-lighters and other stuff.
If your school gives you a paper w/ your locker combination, ID number, etc. on it, you can fold it up and put it in there, too.
Most lockers already come with shelves (usually one or two).
You might need like some kind of an organizer to store and hang stuff.

I would keep extra pens, pencils and paper, as you never know when you or a friend will need it. I would also keep a few quarters in case you need to get a pad or tampon. An extra T-shirt might be nice in case you spill something on it at lunch or art class (as that has happened to me.) It also wouldn't hurt to have a brush in your locker.
On second thought, if you carry a purse (and are allowed to at school; some of the schools I substitute teach in don't allow students to carry purses or bags to class), you might not need to keep quarters, pens, pencils, or a brush in your locker. Still, it might be a good idea to have a backup.
The middle schools I substitute teach for give students lockers. They have a regular locker, a PE locker, and a band locker (if they are in band). A previous poster was correct about some schools not giving students a locker, but the six middle schools I've been in recently provides students with a regular locker.

i would say a locker shelf, some organizers, and some pictures and/or magnets to make it feel homey. Even tho its a pain, clean your locker every 1 to 2 weeks. It will help A BUCH!!
One shelf should be enough. Each middle school year, I always got a new mirror and whiteboard and magnets and pen/pencil holder. But to be honest with you, all I ever used was the shelf. I was always too busy getting to class to ever use any of those other things. Good luck with middle school!
