In the parenthesis, you should put the last name of the author or the first name of the title of the source in quotes, a comma, then the year that it was published, another comma, and the page number). As for the second question, you do not need the author's name for the actual chapter. If it is an anthology, then just put the editors' names, and that will suffice. It should look like this:
