yeah, it means when your talking to someone that you understand what they mean.

The saying "Are we on the same page " is used to ask someone if the person has followed the instructions or has understood whatever has been told to him/her. For example
in a class when the lecturer addresses the classes saying- are we there, she/he means that has the class followed whatever has been taught to them at that particular time
That line is not a saying, it's an idiomatic expression...
