Yes, along with some evidence to prove your health in living with dyslexia. This way, it helps the Dr, evaluate your health status and less chances of being misdiagnosed. And there is nothing wrong with telling a Dr about being dyslexia, trust me!
Would hate if a person was actually misdiagnosed over the 10 yr span. I had a friend who was taking the wrong medication for that long, until a good friend stepped and reasoned with her that the medication ain't helping her at all, no one wants to deal with her because of her loud outbursts.
Today she is so MUCH BETTER, with different med, and she is back with her friends n' family-WHEW-10 yrs for nothing?

Yes I think that you should tell your doctor that you have dyslexia. Not that he would necessarily change anything...just that if he had some helpful advice on how to live with dyslexia and he can make sure that you know what amounts of medications to take if you take medications.
I have a sister-in-law that I help make sure she knows dosages. If she has a question and I'm not there though she either asks the doctor or the local pharmacy. They have went as far to show her by putting it in the syringe themselves. (It is for my four year old nephew, who has heart problems.)
