Always start with the curriculum framework strands/standard that you will cover. Then list your objective for the lesson.
Write the tasks that will be performed by the students in class and lastly the homework that you will give. That is how I do it.

Take it step by step. Each lesson should have some / all of the following elements, although you obviously can't do them all every day.
1) warm-up
2) introduce or review
3) direct instruction, students take notes
4) students read and take notes
5) students do
6) homework
7) assess
