It means 'the action of considering something to be worth no more than the value of a fig', so if I say, 'I floccinoccinihilipiliphicate you', that is a rather unusual insult. The second-longest word in the dictionary is antidisestablishmentarianism, which has something to do with the Christian church. In fact there are longer words which are medical terms, but they don't really count as English - you can't use them unless you've got the disease!
